What is Whole-Body Cold Therapy?
When you hear "cold therapy", you might first think of an ice pack on a swollen knee. Whole-body cold therapy is a very different experience. Here at our studio in Memmingen, it involves spending about three minutes in a specially designed chamber where the air is cooled to a stable -110 °C. Unlike an ice pack, which provides deep, penetrating cold to a small area, the cold in the chamber is a dry, gaseous cold that primarily affects the surface of your skin.
The technology used is a vortex cooling system, which ensures the temperature is consistent and the air is free from moisture, making the extreme cold tolerable for a short period. The principle is to trigger a powerful physiological response by exposing the entire surface of the skin to this intense stimulus. The body's thermoreceptors send urgent signals to the brain, initiating a cascade of reactions designed to protect the core temperature and respond to the extreme environment. It is these systemic reactions that are being studied for their potential to support recovery and wellbeing [7, 8]. The application is brief, systemic, and focused on stimulating a response rather than deeply freezing tissue.
Your First Session: A Practical Walkthrough
The idea of stepping into a chamber at -110 °C can seem daunting, so it helps to know exactly what to expect. Your experience is our priority, and the process is calm and fully supervised.
When you arrive at our studio, you will first have a detailed consultation to ensure the application is appropriate for you. We will review your health history and any contraindications, especially those relevant to a post-surgical state. You must have clearance from your doctor before your first session.
You will then change into appropriate attire. For whole-body cold therapy, this means minimal clothing to expose as much skin as possible to the cold air. We provide you with dry socks, clogs, gloves, a headband to cover your ears, and a surgical mask to warm the air you breathe. You will wear your own swimwear or underwear, which must be completely dry. All jewellery and piercings must be removed.
The cold chamber itself consists of two or more sections. You will first step into a pre-chamber, typically set to around -60 °C, for about 30 seconds. This helps your body begin to acclimatise to the cold and prepares you for the main chamber.
Then, you step into the main chamber at -110 °C. You will be encouraged to walk around slowly and keep your hands moving gently. You are in constant visual and verbal contact with our trained staff throughout the entire session. The cold is intense but very dry, often described as a prickly or tingling sensation on the skin rather than the wet, biting cold of a winter's day in the Allgäu. The vortex system keeps the cold air circulating gently. After two to three minutes, your session is over, and you step out.
Immediately afterwards, you will likely feel a rush of warmth as blood flows back to the skin's surface. Many people report a pleasant tingling sensation and a feeling of lightness or invigoration. Your skin temperature, which drops significantly during the session [5], quickly returns to normal and even overshoots slightly as the body rewarms. Following the session, some gentle movement on a stationary bike or cross trainer for a few minutes can help enhance this circulatory response.
How Cold Therapy May Support Post-Surgical Recovery
While your medical treatments address the root causes of healing, whole-body cold therapy may offer supportive benefits for managing the symptoms that can make recovery so challenging. The potential mechanisms are linked to the body's powerful reaction to the extreme cold stimulus.

Research in related fields, such as inflammatory rheumatic diseases and athletic recovery, has investigated changes in inflammatory markers in the blood following sessions of whole-body cold [2, 8]. For someone recovering from surgery, a more balanced inflammatory response could translate to more manageable levels of swelling and discomfort, potentially making it easier to engage with physiotherapy.
Pain perception may also be influenced. The cold stimulus is a powerful sensory input that travels along nerve fibres to the brain. This can have a temporary analgesic, or pain-relieving, effect. The theory is that the intense cold signals can "gate" or override the pain signals travelling along the same nerve pathways. Furthermore, the cold can slow nerve conduction velocity, which may further contribute to a reduction in perceived pain for a period after the session. Many of our guests report feeling a temporary window of relief, which they can use to perform their rehabilitation exercises with greater comfort and a better range of motion.
Finally, the strong circulatory response prompted by the session is of interest. The initial, brief constriction of blood vessels in the skin and extremities (vasoconstriction) is followed by a significant rebound dilation (vasodilation) after you leave the chamber. This process can be observed thermographically as a rapid rewarming of the skin [5]. This powerful flushing of the circulatory system may help in reducing oedema, or swelling, by supporting the movement of fluid away from the tissues and back into the lymphatic and circulatory systems.
What the Research Shows
The use of whole-body cold therapy is an area of active scientific inquiry. While a large body of high-quality research specifically on post-surgical patients is still developing, studies in related areas provide some interesting, albeit preliminary, insights. It is crucial to approach this research with a clear understanding of its context and limitations.
One area of focus has been the influence of cold application on inflammation. A systematic review from 2014 looked at cryotherapy in the context of inflammatory rheumatic diseases [2]. While these are chronic conditions and not the same as acute post-surgical inflammation, they share the common factor of an active inflammatory process. The authors of the review concluded that whole-body cryotherapy seemed to show beneficial effects on pain and inflammation in these patients, but they also stressed the need for more robust, controlled studies to confirm these findings. Similarly, a comprehensive 2017 review on the use of whole-body cold therapy in athletes noted that many studies suggest it can modulate inflammatory and immune responses after strenuous exercise [8]. Researchers have measured specific markers, such as interleukin-6 (IL-6), a protein involved in inflammation. One study on healthy men undergoing a series of ten sessions observed changes in IL-6 levels, suggesting a potential systemic anti-inflammatory effect [4].
The physiological reaction of the body has been well documented. For instance, thermographic studies have precisely mapped the skin's temperature changes during and after a session. One such study showed a rapid and significant drop in mean skin temperature during the three-minute exposure, followed by a rapid rewarming period afterwards, confirming the powerful vascular response that users experience [5].
It is essential to be cautious when interpreting these results. As critical appraisals of the existing evidence point out, many studies in this field have limitations [7]. These can include small sample sizes, short observation periods, and a lack of a true placebo or control group, which is notoriously difficult in this type of research. For example, it is impossible to "blind" a participant to whether they are receiving a -110 °C treatment or not. Furthermore, much of the research has been conducted on athletes or healthy individuals, and the findings may not be directly transferable to a post-surgical population, who have different underlying physiological states. Therefore, while current research provides a plausible basis for the observed effects and supports the ongoing investigation of whole-body cold therapy, it does not constitute proof of efficacy for post-surgical recovery. It suggests potential mechanisms that may be at play, which align with the experiences reported by many individuals.
Safety and Your Recovery Journey
Your safety is the paramount concern. Whole-body cold therapy is a non-medical wellness application, but it is a powerful one, and it is not suitable for everyone. For individuals recovering from surgery, careful consideration and medical clearance are non-negotiable.
You must not use a cold chamber if you have any of the following contraindications:
- Uncontrolled high blood pressure
- Recent heart attack or stroke
- Unstable angina pectoris
- Peripheral arterial occlusive disease
- Deep vein thrombosis
- Severe anaemia
- Cold allergy (cold urticaria)
- Open wounds or unhealed surgical incisions

Frequently Asked Questions
When can I start using cold therapy after my surgery?
