While medical guidance is the essential first step, many people seek complementary methods to manage their discomfort and support their body's recovery. Understanding Your Shoulder: A Complex Joint
To appreciate why your shoulder can be so prone to problems, it helps to understand its remarkable design. Unlike the stable, weight-bearing hip joint, the shoulder is built for an incredible range of motion. It allows you to throw a ball, swim, and lift your arms above your head. This freedom, however, comes at the cost of stability.
Your shoulder is a ball-and-socket joint. The "ball" is the top of your upper arm bone (the humerus), which fits into a shallow "socket" (the glenoid) on your shoulder blade. Surrounding this joint is a group of four muscles and their tendons known as the rotator cuff. This cuff is crucial; it holds the head of the humerus in the socket and controls the arm's rotation and elevation.
In addition to the rotator cuff, there are ligaments that provide static stability, and a fluid-filled sac called the bursa, which acts as a cushion to reduce friction between the bones and tendons. When any of these structures,the muscles, tendons, ligaments, or bursa,become inflamed, injured, or worn down, the result is pain and restricted movement.
Common Causes of Shoulder Discomfort
Shoulder pain rarely appears without a reason, even if that reason is not immediately obvious. The causes can be broadly grouped into several categories.
Overuse and Repetitive Strain: This is perhaps the most frequent culprit in our modern world. Activities that involve repetitive overhead motions can gradually irritate and inflame the shoulder structures. This includes:
- Desk work with poor posture, where the shoulders are hunched forward for hours.
- Manual labour involving lifting or overhead work.
- Sports such as swimming, tennis, or weightlifting.
- Hobbies like painting or gardening.
This type of strain often leads to conditions like tendinitis (inflammation of a tendon) or bursitis (inflammation of the bursa).
Acute Injury: A sudden event can cause immediate and significant damage. A fall onto an outstretched arm, a direct blow to the shoulder, or a sudden, forceful lift can lead to a torn rotator cuff, a dislocation of the joint, or a fracture. These injuries are typically accompanied by severe pain, swelling, and an inability to move the arm.
Inflammatory Conditions: Sometimes, shoulder pain is a symptom of a wider, systemic issue. Inflammatory rheumatic diseases, such as rheumatoid arthritis or axial spondyloarthritis, can affect the shoulder joint, causing pain, stiffness, and progressive damage [1, 2]. Osteoarthritis, the "wear and tear" form of arthritis, can also develop in the shoulder joint over time, as the protective cartilage thins and wears away.
Frozen Shoulder (Adhesive Capsulitis): This is a particularly perplexing condition where the connective tissue capsule surrounding the shoulder joint thickens and tightens, severely restricting movement. Its onset can be gradual and its exact cause is not always clear, though it sometimes follows an injury or a period of immobility.
The Path to a Diagnosis: When to See a Doctor
Given the complexity of the shoulder joint, self-diagnosing is not advisable. Persistent pain, weakness, a reduced range of motion, or any pain resulting from a specific injury warrants a visit to your general practitioner or an orthopaedic specialist. They are the only ones who can provide an accurate diagnosis and prescribe a medical treatment plan.
A medical assessment will typically involve:
- A detailed discussion of your symptoms, your activities, and your medical history.
- A physical examination to assess your range of motion, strength, and the specific location of the pain.
- Imaging tests may be required to get a clearer picture of the inside of your shoulder. An X-ray can reveal bone issues like fractures or arthritis, while an MRI or ultrasound can provide detailed images of the soft tissues, such as the rotator cuff tendons and bursa.

Established Medical Treatments for Shoulder Pain
Once a diagnosis is made, your doctor will recommend a treatment plan tailored to your specific condition. The approach is often staggered, starting with the least invasive options.
The first line of defence is usually conservative management. This includes the well-known RICE principle: Rest, Ice, Compression, and Elevation. Rest gives the inflamed tissues time to calm down. Applying local ice packs can help reduce swelling and numb pain.
Your doctor may recommend over-the-counter non-steroidal anti-inflammatory drugs (NSAIDs) to manage pain and inflammation.
Physiotherapy is a cornerstone of shoulder rehabilitation. A qualified physiotherapist will guide you through a programme of specific exercises designed to restore mobility, strengthen the rotator cuff and surrounding muscles, and improve your posture and movement patterns to prevent recurrence.
If conservative measures do not provide sufficient relief, your doctor might suggest a corticosteroid injection. This involves injecting a powerful anti-inflammatory medication directly into the affected area, such as the bursa or the joint space, to provide rapid, though often temporary, relief from inflammation and pain.
In a small number of cases, particularly for severe rotator cuff tears or advanced arthritis that does not respond to other treatments, surgery may be the final option.

The principle is based on the body's physiological reaction to extreme cold. When you expose your body to a temperature of -110 °C for a very short period, around three minutes, it triggers a powerful systemic response. This is very different from a local ice pack, which primarily affects the tissues directly underneath it.
Many guests with persistent shoulder discomfort use the cold chamber to help manage the sensation of pain and inflammation. The intense cold can have an analgesic, or pain-relieving, effect by slowing the speed at which nerve cells transmit pain signals. Furthermore, the body's response to the cold involves changes in circulation and the release of various biochemicals, a process that is being explored by researchers for its potential anti-inflammatory effects [4]. After a session, as your body rapidly re-warms, you experience a surge of blood flow, which can leave you feeling invigorated and may help with muscle stiffness.
Your First Session: What to Expect in the Cold Chamber
The idea of stepping into a -110 °C environment can seem daunting, but the experience is quick, dry, and fully supervised. Your first visit to our studio in Memmingen begins with a conversation. We will review your health questionnaire, discuss any contraindications, and answer all your questions.
For the session itself, you will change into minimal, dry clothing, such as swimwear or shorts and a top. We provide you with socks, clogs, gloves, a headband to cover your ears, and a face mask to ensure you are protected and comfortable.
Our chamber uses a two-stage process. You will first spend a few seconds in a pre-chamber set to around -60 °C. This helps your body acclimatise before you step into the main chamber at -110 °C. The air inside is completely dry, which is why the cold feels crisp and tolerable, rather than damp and chilling like a winter's day in the Allgäu.
You will be in the main chamber for a maximum of three minutes. You are encouraged to walk slowly in a circle and breathe normally. Our trained staff are in constant visual and verbal contact with you throughout the entire session. The moment it is over, you step out, and the feeling is remarkable. You might feel a tingling sensation on your skin as your circulation returns with a rush, bringing a deep sense of warmth and often an immediate feeling of alertness and wellbeing.
What the research shows
Scientific interest in whole-body cold therapy has grown, particularly in the contexts of sports recovery and managing inflammatory conditions. While research is ongoing and many questions remain, several studies offer insights into its potential mechanisms and effects. It is crucial to interpret these findings with caution.
A systematic review published in 2014 looked at the use of cryotherapy for inflammatory rheumatic diseases [2]. The authors analysed several studies and found that whole-body cold therapy appeared to produce a temporary analgesic effect. In patients with conditions like rheumatoid arthritis and ankylosing spondylitis, which can both affect the shoulder, some studies reported reductions in disease activity markers after a series of sessions [2]. However, the review also highlighted that many of the included studies were small and called for larger, more methodologically robust trials to confirm these effects.
Another comprehensive review focused on the application of whole-body cold therapy in athletes, a group often dealing with overuse injuries akin to some types of shoulder pain [4]. The researchers noted that exposure to extreme cold seems to influence the inflammatory response. Some studies observed a decrease in pro-inflammatory cytokines, which are molecules that signal and promote inflammation, and an increase in anti-inflammatory cytokines after sessions [4, 7]. This suggests a potential mechanism for how whole-body cold could help modulate inflammation following intense physical exertion. However, the scientific community rightly maintains a critical perspective. A 2014 critical appraisal of the evidence base pointed out significant limitations in the existing research [3]. Many studies lack a proper control or placebo group, making it difficult to distinguish the specific effects of the cold from a potential placebo effect. The optimal number of sessions, their duration, and the ideal temperature are still subjects of investigation. Furthermore, individual responses can vary significantly based on factors like body composition and sex, which influence how quickly and deeply the skin cools during a session [8]. Therefore, while the existing data on inflammation and pain modulation are intriguing and point towards plausible benefits, they must be considered preliminary. The research does not support any claims of a cure but suggests that whole-body cold therapy may be a useful tool for symptom management as part of a broader health strategy.
Important Considerations and Contraindications for Cold Therapy
Your safety is our highest priority. Whole-body cold therapy is a powerful stimulus and is not suitable for everyone. It is essential to be aware of the absolute contraindications, which include:
- Uncontrolled high blood pressure
- Heart attack within the last six months
- Decompensated diseases of the cardiovascular and respiratory system
- Unstable angina pectoris
- Peripheral arterial occlusive disease
- Presence of a pacemaker
- Venous thrombosis
- Acute febrile illness
- Severe anaemia
- Cold allergy (cold urticaria)
- Pregnancy
- Raynaud's syndrome
Specifically regarding your shoulder, it is important not to use the cold chamber for a very recent, acute injury with open wounds or undiagnosed severe pain. Whole-body cold therapy is intended to support the management of chronic or recovery-phase conditions, always in consultation with your doctor. It is a wellness application, not a medical procedure.

How often should I use the cold chamber for shoulder pain?
This is highly individual and depends on your goals and the nature of your discomfort. Many people start with a series of sessions in relatively quick succession, perhaps 5 to 10 sessions over one or two weeks, to gauge their body's response. Afterwards, they may transition to a maintenance schedule of once or twice a week as part of their ongoing routine.
Is whole-body cold therapy a substitute for my physiotherapy?
Absolutely not. Whole-body cold therapy should be seen as a complementary modality. Physiotherapy is crucial for addressing the root causes of many shoulder problems by improving strength, mobility, and mechanics. The cold application may help manage pain and stiffness, potentially making it easier for you to perform your prescribed exercises.
Can I use the cold chamber after shoulder surgery?
You must wait until your surgical wounds are fully healed and your surgeon or physiotherapist has given you explicit clearance to do so. Introducing any new stimulus during the delicate post-operative phase should only be done with medical approval.
Will the session be painful?
The sensation is one of intense cold on the surface of your skin, but it is generally not described as painful due to the dryness of the air. The three-minute duration is short, and our staff monitor you constantly to ensure you feel secure. Any discomfort ceases the moment you exit the chamber.
What is the main difference between whole-body cold therapy and just using an ice pack?
An ice pack provides localised cold, primarily affecting the superficial tissues in a small area. Whole-body cold therapy exposes the entire body to a brief, extreme temperature drop. This is believed to trigger a systemic, body-wide neuro-reflexive and hormonal response that is different in scale and nature from local icing.
Can I come for a session if my shoulder just feels a bit stiff from work?
Yes, many of our guests use the cold chamber for general wellness and recovery.
